Case Management
Case Management services feature professional nurses who have added training in coordination of health care, collaboration of insurance benefits, fiscal analysis and monitoring. They are nurses who have a deep desire to see patients receive high-quality care without resulting in great financial burdens to the patient as well as the organization. Discharge Planning
Hospitalization for cancer treatment may require follow-up care at home or in a rehabilitation or sub-acute setting for optimal recovery. Some forms of treatment can be given at home to reduce the number or length of hospital admissions. A clinical social worker and case manager work together to ensure that plans for care throughout the continuum of treatment are coordinated with the medical team, insurance company, preferred providers and patient/caregiver. Lodging
The distance between your home and Moffitt Cancer Center may make travel for extended courses of treatment and follow-up medical appointments prohibitive. A lodging coordinator in the Psychosocial & Palliative Care Program helps patients and families make arrangements for local lodging. Hotels that offer a reduced reservation rate for Moffitt patients and families are listed at this link.
