Self-Referral
Option 1: Patients may schedule a visit, consultation or second opinion, by contacting the New Patient Appointment Center at 1-888-860-2778 or 813-745-3980. A patient representative will schedule your appointment and help you gather all of the appropriate information, such as medical records, X-rays, pathology slides and test results, before you arrive.
Option 2: Complete the online New Patient Pre-Registration form, which will activate your new patient appointment process. Our New Patient Scheduling Specialist will call you directly to collect additional information required before confirming your appointment.
Physician Referral
Option 1: Physicians may refer patients by calling the New Patient Appointment Center, noted above. The Oncology Consultation Service, 1-800-456-7424, also works with physicians who have questions about referring a patient or arranging a physician-to-physician consultation. Physicians may also call Moffitt physicians directly to discuss individual patient cases. To refer a patient for possible participation in a clinical research study, contact the Clinical Research Office at 813-745-4106.
Option 2: If you are a physician or a physician representative, complete the online Referring Physician form. Patients will be contacted in order to review insurance coverage and obtain additional demographic information. Medical and financial eligibility need to be established prior to confirming an appointment. All online Referral Forms will receive a response within one business day.
