What a Patient Should Expect

On your first visit to the Palliative Care Clinic you will be asked to complete a questionnaire describing your symptoms and to provide information on all current medications. You may be seen by many providers, including:  Medical Assistant, Nurse, Doctor of Pharmacy, Fellow Physician, Attending Physician, Advanced Registered Nurse Practitioner and Social Worker.

On your first visit to the Psychosocial Clinic, you will be asked to complete a questionnaire describing your symptoms. You may also see one of our clinical psychologists or psychiatrists.

Make an Appointment

Option 1:  Patients who are new to Moffitt Cancer Center should call the New Patient Appointment Center at 813-745-3980 or 1-888-860-2778 (during normal business hours).  

Option 2:  You also can start your registration process by completing the New Patient Preregistration form, which will activate your new patient appointment process. Our New Patient Scheduling Specialist will call you directly to collect additional information required before confirming your appointment.

Patients who have been seen previously at Moffitt Cancer Center can call the Palliative Care Clinic or Psychosocial Clinic directly, or the referring clinic can call 813-745-4630.
